Oʻahu DEM Encourages Residents to Report Storm Damage

The City and County of Honolulu has released an online form for Oʻahu residents to self-report damage to their homes from FLASH FLOODING on Oʻahu.

The purpose of this form is to collect information that will help county officials and our partners better understand the damage that occurred and impacts on our community.

To submit residential damage assessment information please go to: www.honolulu.gov/homedamage

When submitting the form, please note that the purpose of this form is only to collect information that will help local officials and our non-profit partners understand the damage that occurred and impacts on the community

The self-reporting tool is NOT an application for relief programs or services.

  • Reporting damage is a voluntary activity and is not a substitute for reporting damage to your insurance agency, nor does it guarantee disaster relief assistance.
  • Please submit one survey per affected household. If living with roommates, please submit one per roommate.
  • This form is for damage that occurred on O‘ahu only.
